Who is eligible to participate in the BBR program?

The following diagnosis are generally approved for participation in the Breathe Better Rehab telehealth rehabilitation program:

  1. COPD (Chronic Obstructive Pulmonary Disorders):
    Listed in the following levels of severity according to GOLD guidelines:
    • Moderate
    • Severe
    • Very Severe
  2. Chronic lung diseases:
    • Bronchiectasis
    • Interstitial Lung Disease (ILD)
    • Pulmonary Fibrosis (PF)
    • Cystic Fibrosis (CF)
  3. Post-COVID-19:
    • Pulmonary Complications
    • Long COVID Respiratory Symptoms
  4. Cardio-Pulmonary Disorders:
    • Pulmonary Hypertension
  5. Lung Transplant:
    1. Preparation Prehabilitation
    2. After Care Rehabilitation Following Lung Transplant
  6. Endobronchial Valve (EBV):
    1. Preparation Prehabilitation Prior To Procedure
    2. After Care Rehabilitation Following Procedure

Upon receipt of your provider’s referral, our insurance department will verify your coverage and inform you of any co-pays you may be responsible for.

Breathe Better Rehab participates with most major insurances, including:

Our insurance department is happy to make an inquiry into your plan and coverage to verify participation.

  1. Healthcare Provider Referral
    Your healthcare provider will send a referral to Breathe Better Rehab telehealth program on your behalf. You may also contact us to have us reach out to your health care provider to initiate a referral.
  2. Insurance Authorization
    Upon receipt of your provider’s referral, our insurance department will verify your coverage and inform you of any co-pays you may be responsible for.
  3. Initial Consultation
    Your initial consultation with our respiratory health care professional will review your provider’s referral information, discuss pertinent matters related to pulmonary rehab and share their findings with our clinical team.
  4. Session Scheduling
    Your interactive telehealth rehab program class schedule will be coordinated with our scheduling department. Your patient kit that includes items for participation will be shipped to you.

The initial consultation with the respiratory physician is 30 minutes long. Follow up consultations halfway through the program and upon discharge from the program are also 30 minutes.

The rehabilitation program is up to 36 sessions based on medical necessity. Each session is 45 minutes active time with the licensed respiratory therapist and patient care coordinator.

The interactive sessions are led by your assigned, licensed respiratory therapist (RRT). The respiratory therapist is a trained healthcare professional with experience working with patients diagnosed with a variety of lung diseases. Their training and background also includes working with patients in rehabilitation exercises including cardio-pulmonary, muscular strengthening and pulmonary endurance.

The sessions are assisted and monitored by a patient care coordinator medical assistant. Their role includes helping you to check in for class, addressing technical/connection issues, collecting patient vitals, monitoring class activity and assist the respiratory therapist in reporting.

Your doctor, nurse practitioner or physician assistant will be the one to help you determine if telehealth pulmonary rehabilitation may be a good option based on your health status and factors that include: access to an on-site program, transportation issues, personal mobility challenges, and limitations.

Your healthcare provider will be notified:

  • Upon your acceptance into the program
  • At the 30 day progress reporting date
  • Upon your graduation discharge
  • Immediately if there are any issues affecting your overall health status or ability to participate in the program
  • Strength and conditioning exercises
  • Cardiovascular and cardio-pulmonary exercises
  • Controlled breathing techniques such diaphragmatic breathing/pursed lip breathing
  • Relaxation techniques
  • Learning positions that help relieve shortness of breath

After you schedule your initial consultation with one of our patient coordinators, you will receive an email from Breathe Better Rehab with a link to your appointment. No download is required! You can connect using your smartphone, iPad, tablet, laptop, or desktop computer.

To join the appointment, click the blue “Start Appointment” button in the email. A new browser page will automatically open, and you will be prompted to type in your name. Once you type in your name, click the orange “Continue” button. You may see an orange button that says “Request Permissions” that will ask you to allow your device to access the camera and microphone. Please click allow.

You will then be taken to a screen where you should be able to see yourself in the camera! Please click the button that says “Knock” to alert the provider that you are waiting to be let into the appointment. Once the provider lets you into the meeting, you will be able to see both the provider and yourself on the screen.

At the conclusion of your time in the program, you will be eligible to enroll in our maintenance program. The maintenance program is designed to help you maintain your progress as well as assist you in continuing your improvement and optimize quality of life. You can choose to participate in 4 to 8 sessions a month at a fixed cost. The maintenance program is not covered by insurance plans and will be billed directly to you.
